INSURANCE COVERAGE

All Cushions and Overlays are covered by OSI's acceptable insurances:

     •Medicare Part B
     •Evercare
     •Senior Whole Health

• Cushions and mattress overlays are covered 100% for residents in a Skilled Nursing Facility only by Evercare and Senior Whole Health (not Medicare Part B).

• Cushions and mattress overlays are NOT covered by Medicare if a resident is at a Skilled Nursing Facility long

term; however, they are covered and can be provided within 48 hours of the resident being discharged home.

• Cushions and mattress overlays are covered 100% for patients who reside at home or in an assisted living facility by ALL of Orthotic Solution, Inc.'s accepted insurances: Medicare Part B, Evercare and Senior Whole

Health.

• Orthotic Solutions, Inc. is responsible for obtaining Physician orders and all necessary paperwork AND does all of the billing to insurance companies.

INSURANCE CRITERIA

WHEELCHAIR SEAT CUSHIONS

     • The patient/resident must have a wheelchair AND

     • Has EITHER of the following:

             • Current pressure ulcer OR past history of a pressure ulcer on the area of contact with the seating surface                 (buttock, lower back and/or hip) OR

• Absent or impaired sensation in the area of contact with the seating surface OR inability to carry out a functional weight shift due to a specific diagnosis (such as Alzheimer's, Parkinson's, MS and others).

MATTRESS OVERLAY

• The patient/resident is completely immobile (cannot make changes in body position without assistance), OR

     • The patient/resident has limited mobility (cannot independently make changes in body position significant enough         to alleviate pressure) AND at least ONE of the following:

             • Any stage pressure ulcer on the trunk or pelvis

             • Impaired nutritional status

             • Fecal or urinary incontinence

             • Altered sensory perception

             • Compromised circulatory status

The EX CEL Pressure Reduction Gel/Foam Mattress Overlay is your first line of defense and response for the prevention and treatment of pressure sores:

Gel mattress overlay protects skin integrity with 2" gel packs.
